To avoid any misunderstanding concerning fees, you will receive an estimate of the proposed services before any surgical treatment. We offer several payment options, outlined below. If you do not have insurance coverage for the services rendered, we appreciate full payment at the time of service. However, we are sensitive to the fact that some patients without insurance may not be able to pay cash for their treatment; therefore, we have staff available to discuss payment of your balance due.
We accept the following payment options:
- Credit Card (Visa, MasterCard, Discover, and CareCredit)
- Medical/Dental or other insurance that will cover your treatment
Financial and Billing Policy
For us to bill your insurance company accurately, you are required to provide us with all of your insurance information, both medical and dental. Your insurance policy is a contract between you and your insurance company, and you are ultimately responsible for making sure that all covered benefits are paid. If you do not provide us with the required billing information that we requested within seven days of the request, your balance will then be considered due in full from you. If we have not received a payment from your insurance carrier after 90 days, we expect payment in full from you. Therefore, we ask that you follow up on any claims that have not been paid within 60 days of treatment being rendered.
You will receive a statement each month informing you of any unpaid balance. Also, finance charges will begin to accrue on the 91st day on any unpaid balance, regardless of whether or not your insurance company has made a partial payment. In the event the balance remains unpaid at this time, your account will be forwarded to a designated collection agency. You will then be responsible for the unpaid balance as well as any service fees associated with the collection process.
Note: Even though a Pre-Treatment Estimate or Prior Authorization may be required by your insurance carrier or requested by you, a response back from the carrier is not a guarantee of payment from them. We are required by law to collect all co-pays at the time services are rendered and the deductible once your insurance has processed your claim.
